The Center for Children with Special Needs is a program of Seattle Children’s Hospital in Seattle. This program unfortunately is no longer funded, but useful resources curated by this program is still accessible on their website.

Early Support for Infants and Toddlers (ESIT)

Administered by the Department of Children, Youth, and Families (DCYF), ESIT directs the coordination of the statewide system of early intervention services for families with children age birth to three that have developmental delays.

Medical Home Partnership Project

The Washington State Medical Home website identifies practical tools and information that support family-centered, coordinated care (e.g. medical homes) for children and youth with special health care needs in Washington. Resources include select diagnosis-specific care guidelines, patient handout and weblinks as well as links to state and community services. There are specific sections for families, physicians and other primary care providers (PCP), as well as for community service providers such as nutritionists whose work supports the family and PCP.

Got Transition

Got Transition® is the federally funded national resource center on health care transition (HCT). Its aim is to improve the transition from pediatric to adult health care through the use of evidence-driven strategies for clinicians and other health care professionals; public health programs; payers and plans; youth and young adults; and parents and caregivers. The website includes FAQs and resources for youth/young adults and parents/caregivers.

Informing Families

Informing Families is a resource provided by the Washington State Developmental Disabilities Council, in partnership with the Developmental Disabilities Administration. This website is a clearinghouse for a wide range of resources for CYSHCN and their families, organized by topic (DDA, assistive technology, advocacy, etc.) and by age group.
